Welcome to the dedicated page for submitting Safety Reports at Hexagone Aviation School. This platform enables you to report, either by name or anonymously, any incident or safety concern, thereby contributing to the continuous improvement of our aviation environment.

Important: Regardless of whether the report is submitted anonymously or by name, all published safety reports will be anonymous. Personal and nominative information will remain strictly confidential and will only be accessible to our school's safety department.

Safety First

The primary objective is to enhance safety by understanding errors and avoiding unnecessary blame. By openly reporting errors and incidents, you contribute to a better comprehension of risks and the implementation of effective preventive measures.

By reporting incidents or risks, you help our organization learn from each situation and continuously improve. The Just Culture enables us to understand why errors occur and to implement solutions to prevent their recurrence.

Shared Responsibility

The Just Culture distinguishes between unintentional errors, at-risk behaviors (such as taking shortcuts), and deliberately reckless behaviors. Only genuinely reckless or intentional behaviors lead to sanctions. Errors or at-risk behaviors are addressed through process improvements.

Types of Safety Reports

At Hexagone Aviation School, anyone in contact with our organization — whether you are a student, administrative staff member, instructor, partner, subcontractor, or supplier — may be required to submit a Safety Report. 

It is essential to report any safety incident or risk as soon as you become aware of it. Reports can be categorized into two types:

  1. Mandatory Reports: for incidents that meet criteria defined by regulations and pose a direct risk to flight safety.
  2. Voluntary Reports: for events or situations not covered by mandatory reporting requirements, but which can nonetheless contribute to safety improvement or prevent future incidents.

1. Mandatory Safety Report

Certain incidents must be reported in accordance with current regulations (EASA Regulation (EU) No 376/2014). These incidents are those that pose a direct risk to flight safety and must be reported within 72 hours of the incident or as soon as you become aware of it.

Examples of Mandatory Reports:

Operational Incidents

Navigation errors, deviations from flight procedures, non-compliance with ATC instructions.

Technical Failures

Equipment anomalies or malfunctions that could affect aircraft airworthiness.

Air Traffic Management Incidents

Aircraft separation violations, errors in navigation services.

Ground Incidents

Any ground event affecting the safety of air operations.

Dangerous Goods Incidents

Incidents related to the transport of dangerous goods.

Cabin Safety

Issues with emergency equipment, cabin pressurization, cabin safety procedures.

Unmanned Aircraft Systems (Drones)

Incidents involving drones that could affect aviation safety.

2. Voluntary Safety Report

The Voluntary Safety Report allows for the reporting of events or observations that do not require mandatory reporting but could contribute to enhancing safety. These reports complement the mandatory reporting system by capturing essential information to prevent future incidents.

Examples of voluntary reports:

Safety Concerns

Any potential risk observed, even if it has not yet caused an incident.

Near Misses

Incidents that could have led to an accident but were narrowly avoided.

Suggestions for Improvement

Ideas or proposals to enhance procedures or equipment.

Unsafe Practices

Observed behaviors or actions that could compromise future safety.

Human Factors

Incidents related to fatigue, miscommunication, or workload overload that could compromise safety.

Environmental Risks

Meteorological or natural conditions that may pose a safety hazard.

How to Submit a Safety Report?

To file a Safety Report, whether mandatory or voluntary, please complete the form below. You may choose to submit your report anonymously or with identification. In all instances, your information will be handled with complete confidentiality, in accordance with our commitment to the Just Culture principle.

You have the option to remain anonymous when submitting this Safety Report. Should you choose to identify yourself, please be aware that only personnel from Hexagone Aviation School's safety department will have access to your personal information. Identification allows the safety department to contact you to collaborate more effectively on the follow-up of your report. This enables us to ask supplementary questions, clarify specific points, or work together to maximize the benefit of your submission and thereby enhance our safety practices. In all cases, your confidentiality will be strictly maintained.
